Helping Hands:  The FAVA Fund

Due to a generous grant awarded to Project FAVA by The Changemakers Program, Project FAVA is pleased to offer financial assistance to those living with a fibro-adipose vascular anomaly (FAVA) and their caretakers through our assistance program, Helping Hands: The FAVA Fund.  Our goal with this fund is to alleviate some of the medical costs associated with FAVA in order to eliminate financial barriers to medical care.        

To qualify for assistance under this program, you or your child must meet these qualifications:

  • Be diagnosed with a fibro-adipose vascular anomaly

  • Demonstrate financial need 

  • Be a member of Project FAVA

Financial assistance requests must be associated with FAVA treatment, medical-related travel, medically needed equipment, or other medical expenses related to FAVA.   

 

Project FAVA has discretion to award up to four grants.   

An enormous thank you to The Changemakers for making this grant possible and believing that affordable medical options should be within the reach of everyone.  

APPLICATION FOR FINANCIAL ASSISTANCE

  • Financial assistance will be determined on a case-by-case basis, depending on need and Project FAVA's ability to assist.

  • Application must demonstrate applicant's financial need. 

  • Please submit proof of FAVA diagnosis, as well as receipts/documentation of expenses incurred equal to the amount you are requesting.  Depending on circumstances and needs, you may be asked to disclose additional information/documentation.  Please do not send any information containing social security numbers.

  • The award limit per family is $500 USD.

Please send to hello@projectfava.org receipts/documentation of:

(1) Diagnosis of fibro-adipose vascular anomaly

(2) Medical expenses incurred equal to the amount you are requesting

(3) Proof of financial need

Your application will not be considered complete until such documents are received.
